One Big Beautiful Bill Act
Cutting What North Carolina Can't Afford to Lose
The One Big Beautiful Bill Act (OBBBA) ripped away billions of dollars in essential healthcare funding from North Carolina – gutting hospital budgets, undermining an already strained system, and putting vulnerable North Carolinians' access to care at risk.
THE FACTS
North Carolina stands to lose $6.6 billion in Medicaid funding annually. More than 600,000 North Carolinians are at risk of losing Medicaid coverage (NCHA).
More than half of North Carolinians oppose OBBBA, because they worry it will lead to coverage loss and cuts to essential care (Heal the System Poll).
THE REALITY FOR HEALTHCARE PROVIDERS AND PATIENTS
The OBBBA’s sweeping federal and state budget cuts – combined with new debt forgiveness mandates and federal work requirements – will devastate hospitals, health systems, and patients across the state.
-
Hospitals are economic anchors and lifelines for the health of their communities. Funding cuts threaten to force hospitals to reduce services, delay needed upgrades, or close units, putting care and jobs at risk for thousands of North Carolinians.
-
New funding reductions and federal work requirements could push thousands off Medicaid – not because they are ineligible, but because of paperwork, red tape, and reporting hurdles. Families who finally gained access through Medicaid expansion could find themselves uninsured once again.
-
Many rural hospitals already operate on razor-thin margins. These cuts could be the final blow – closing emergency rooms, halting maternity care, slashing mental and behavioral health services, and even shutting doors entirely. When a rural hospital closes, entire communities can lose their only local source of medical care.
To offset these impacts, the OBBBA proposes the Rural Health Transformation Program – a national, competitive $50 billion pool. But with the second-largest rural population in the country, North Carolina would have no guarantee of receiving enough funding to protect the nearly 3 million residents who depend on already fragile rural health systems.
Why this Matters to YOU
When hospitals and healthcare providers struggle, every North Carolinian feels it. North Carolinians are worried about the future under OBBBA – 40% cite cuts leading to coverage loss as their top concern (Heal the System Poll). These cuts reach deep into communities – threatening care, jobs, and stability.
-
Working families face fewer options when hospitals cut services.
-
Seniors lose access to specialists and consistent care.
-
Local economies lose jobs, stability, and trusted institutions.
-
Rural residents may lose their only source of medical care, with no alternatives for miles.
This isn’t just a healthcare problem – it’s a community crisis. Every hospital that scales back services and every family that loses coverage chips away at the health and stability of our state.

STOP FINANCIAL HARM TO HOSPITALS
OBBBA has stretched our healthcare system beyond its breaking point. State lawmakers must act to repair the damage, protect hospitals from further financial harm, and safeguard access to essential care. That means restoring funding wherever possible, maintaining stability for hospitals and providers, and strengthening the safety net for those who rely on Medicaid. The future of healthcare in North Carolina requires swift action to repair the damage and keep hospitals, healthcare providers, and Medicaid coverage strong for all North Carolinians.
